When the system becomes the constraint

The person nearest the visible problem often receives the intervention. Sometimes that is correct. Sometimes the system is producing the pattern.

Unclear authority, late information, contradictory measures, social risk and impossible demand can compress expression while leaving capability intact.

Redesign is not an excuse to remove standards. It is the disciplined choice when the role, task or surrounding conditions are badly configured. The test is whether changing the system changes the performance pattern.
